By The Bay Health, a non-profit established in 1975, seeks a Registered Nurse to serve as Hospice Nurse Case Manager. The role provides skilled nursing assessment, planning and care to maximize comfort for patients and families, within interdisciplinary teams and home-based settings.

The schedule is Monday–Friday, 8:30am–5:00pm. Pay range is $65.70–$75.70 hourly, with comprehensive benefits and a supportive work culture.

Formación

• AA or B.S. degree in Nursing from an accredited Nursing School.
• Current license as a registered professional nurse in California.
• Experience in acute care/home health and/or hospice preferred but not required.
• Public Health Nursing certification highly desirable.
• HPNA certification preferred.
• Current California Driver’s License and automobile insurance that meets State of California requirements, if driving a car.

• Coordinate all patient/family services and prioritization of needs, including implementation of the individualized plan of care.

Our spectrum of home-based services now includes Skilled Home Health Care, Palliative Care, Adult Hospice Care and Pediatric Care. Our team approach strives to address practical, social, emotional and spiritual aspects of care, with the goal to maximize quality of life for our patients, caregivers and families.

We are currently seeking a Registered Nurse to serve as a Hospice Nurse Case Manager. This person will be responsible for the provision of skilled nursing assessment, planning and care to maximize the comfort and health of patients and families.

Schedule

Monday – Friday; 8:30am – 5:00pm

Location

Pay Range

$65.70 – $75.70 hourly
