Cassia is seeking an Admissions Specialist to support hospital-system partnerships and coordinate transitions for residents across Minnesota. This is a full-time, primarily remote Home Office role based in Edina, MN, with some in-person field work and occasional weekends.

Qualifications

• Licensed professionals encouraged to apply, licensure not required.

• Experience with senior healthcare systems and admissions coordination preferred.
• Ability to multi-task in a fast-paced environment and make timely decisions.
• Familiarity with Epic, Matrix or other EHR systems is a plus.

Responsibilities

• Coordinate services for new admissions to skilled care.
• Monitor occupancy and support census growth through referrals and admissions.
• Liaise between hospital discharge planners, families and residents.
• Ensure adherence to admissions protocols and safety standards.
• Make admission decisions when appropriate (acceptances/denials).
• Complete full admissions services during transition from hospital to care.
• Develop and maintain data tracking and analytics.
• Share insights with hospital staff, Cassia teams, and families.
• Identify opportunities to improve process efficiency and collaboration.

Job description

Cassia, a nonprofit senior living organization, is seeking an Admissions Specialist to support strong partnerships between hospital systems and our TCU and LTC communities across Minnesota. This is a great opportunity for an experienced professional to bring their expertise to a mission-driven organization dedicated to serving older adults.

Position Type: Full-Time, benefits eligible position is a Home Office position located in Edina, Minnesota, this position is primarily a remote work from home position. Minimal travel is required, as well as some in person field work. M-F with some rotating weekend and afternoon coverage required.

Wage Range: $75,000 - $93,000 Annually depending on experience

Admissions Specialist Responsibilities

• Communicate and Coordinate Services for New Admissions to Skilled Care.
• Monitor occupancy performance and support census growth through effective referral and admissions management.
• Serve as the primary liaison between hospital discharge planners, family, and incoming residents while new residents transitioning into Cassia’s services.
• Ensure adherence to admissions protocols, processes, and standards.
• The Ability to make admission decisions which include but are not limited to admissions acceptance and denials.
• Complete full skilled care admissions services coordinating services and care throughout a resident’s transition from hospital to skilled care communities.
• Ensure that admissions are completed thoroughly and safely.
• Develop and maintain data tracking and analytics as required.
• Communicate key insights and admission/transitional information to hospital staff, Cassia Site Staff, Families, and incoming Residents.
• Identify opportunities for process improvement to enhance operational efficiency and collaboration.
• Work collectively and collaboratively with team members.

Admission Specialist Qualifications

• Licensed Social Workers, LPNs, RNs, and other transitional care professionals are encouraged to apply, but licensure is not required.
