Job summary

BAYADA at BAYhealth Admissions Registered Nurse is a full-time role performing home health visits with adult patients across Kent County, Delaware. You will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to help clients remain at home.

You will follow care plans, administer medications, perform wound care and assessments, document changes, and coordinate with Clinical or Client Services Managers.

Qualifications

• Current license as a Registered Nurse in DE.
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Home health experience preferred but not required.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.

Responsibilities

• Follow a designated care plan in accordance with patient needs.
• Make home visits to clients in designated geographic territories.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to the Clinical or Client Services Manager.
• Follow up with, execute, and properly document doctors' orders.
• Perform client assessments as necessary.
