Job summary

BAYADA Home Health Care, Inc. is seeking a Full-Time Admissions Registered Nurse to perform home health visits for adult patients in Kent County, Delaware. You will integrate into a multi-disciplinary team delivering rehabilitative care and skilled nursing to enable patients to remain at home.

The RN will follow care plans, administer medications, and document orders. Travel to assigned cases and EMR documentation are required.

Qualifications

• Current DE Registered Nurse license.
• Minimum of one year of clinical (medical/surgical) experience.
• Home health experience is preferred but not required.
• Graduate of an accredited nursing program with transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal and communication skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Follow a designated care plan to meet patient needs.
• Make home visits to clients in designated territory.
• Administer medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to supervisor.
• Follow doctors' orders and document observations accurately.
• Conduct client assessments and coordinate case management.
• Document care using a tablet and EMR system.

Full Time opportunity performing home health visits with adult patients. This office serves clients throughout Kent County.

Registered Nurse (RN) Responsibilities

• Follow a designated care plan in accordance with patient's needs
• Make home visits to clients in designated geographic territories
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures
• Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
• Follow up with, execute, and properly document doctors' orders
• Perform client assessments as necessary
• Case management and coordination
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a Registered Nurse (RN)

• A current license as a Registered Nurse in DE.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information

Base Pay, depending upon qualifications, status, and subject to negotiation:

$62 - $67 per point ($96,720 - $104,520 annual earning potential)
