Job summary

BAYADA at BAYhealth is seeking a Registered Nurse (RN) for full-time or part-time home health visits, assessments, and admissions with adult patients in Delaware. You will join a multidisciplinary team delivering rehabilitative care and skilled nursing so clients can remain at home.

Responsibilities include following care plans, administering medications, wound care, and documenting observations using a touch pad tablet.

Qualifications

• Current Delaware RN license required.
• Minimum of one year of clinical (medical/surgical) experience.
• Home health experience preferred but not required.
• Graduation from an accredited nursing program with transcript or diploma.

Responsibilities

• Follow designated care plans based on patient needs.
• Make home visits to clients in Dover, Camden and Magnolia areas.
• Administer medications, wound care, treatments and procedures.
• Monitor patient conditions and report changes to the manager.
• Document doctors' orders and assessments accurately.
• Perform client assessments and assist with case management.
