What You Must Have:

• Current State Registered Nurse License
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
• Two years of experience as an RN in a healthcare/clinical setting, hospice preferred.

What You Will Do:

• Consult with and educate the patient / family regarding end-of-life care.
• Conduct initial meeting with the patient and family over hospice service and philosophy and obtain consents for care.
• Develop an initial care plan that establishes immediate goals and interventions, based on nursing diagnosis. Includes the patient and the family in the planning process.
• Initiate communication with attending physicians, hospice staff members, and other facilities as needed to coordinate optimal care and use of resources for patient / family members.
