Empath Health's Tidewell Hospice unit is seeking an Admissions RN to assess eligibility and advocate for appropriate access across care settings. You will complete admissions per program guidelines, ensuring patient-centered care and effective coordination with the care team.

Qualifications

• RN license in Florida and current CPR certification.
• Associate's degree in Nursing or equivalent required.
• Experience in hospice or home health settings preferred.

Responsibilities

• Assess eligibility for Empath Health Network of Care and advocate for access to appropriate care.
• Complete admission process per care program guidelines and patient needs.
• Demonstrate compassionate service and strong relationship-building with patients, families, and internal teams.
• Ensure compliance with regulatory standards and EMR billing requirements.
