AdamsPlace seeks an Admissions RN to coordinate all aspects of a patient’s admission from intake to program admission in Lawrenceburg, TN. You will educate families about hospice services and advocate for a positive experience with hospitals, nursing homes, and referral sources.

Responsibilities include gathering intake information, completing admissions paperwork, coordinating follow-up with referral sources, and serving as a clinical resource for marketing staff.

Qualifications

• Must be a Registered Nurse licensed in the state of operation.
• Knowledgeable regarding high quality hospice care and able to provide patient care.
• Must possess good communication, organizational and interpersonal skills, able to work with a wide variety of people.

Responsibilities

• Communicate hospice services to prospective patients, families and referral sources.
• Gather initial intake information and confirm benefits with corporate referral center.
• Educate referral sources, patients’ families and community regarding hospice utilization and benefits.
• Complete admissions paperwork using information obtained from patients, responsible party, referral source, etc.
• Coordinate follow up with referral sources.
• Conduct clinical evaluations and serve as clinical resource for marketing staff; completes preliminary admissions paperwork; personally conducts all admissions when available; coordinates admissions when more than one is pending.
• Follow through to ensure admissions process is implemented on a timely basis.

Our Admissions RN coordinates all aspects of a patient’s admission, and ensures a smooth and timely process from the initial intake to program admission. The Admissions RN assists in educating the family regarding hospice services and ensures the patient is comfortable throughout the admission. Works in a collaborative and persuasive manner with the hospital, nursing home, or other admissions staff, to help ensure they have a positive experience.

• Must possess good communication, organizational and interpersonal skills, be outgoing, perceptive and self motivated with the ability to work harmoniously with a wide variety of people.


• Prior management experience preferred. Must be able to function efficiently in a stressful work environment.
