Qualifications

• Must be a RN licensed in the state of operation (TN).
• Knowledge of high quality hospice care and ability to provide patient care.

Responsibilities

• Communicate hospice services to prospective patients, families and referral sources.
• Gather intake information and confirm benefits with referral center.
• Educate referral sources, patients’ families and community about hospice utilization and benefits.
• Complete admissions paperwork from patients and responsible party.
• Coordinate follow up with referral sources.
• Conduct admissions and serve as clinical resource for marketing staff; manage admissions Timelines.
