Job summary

A prominent Home Health Care company in Albuquerque, NM, is seeking a full-time Admissions Registered Nurse to join its team. The role involves coordinating patient care and assessing individual needs, requiring 1 year of Home Health experience and knowledge of OASIS. Qualifications

• Must have Home Health experience and knowledge of OASIS.

• 1 year of Home Health experience is required.

Responsibilities

• Obtain data and assess necessary patient information.
• Coordinate care with interdisciplinary team.
• Re-assess patient needs and update the plan of care.
• Perform therapeutic, preventative, and rehabilitative nursing procedures.
• Maintain accurate patient records.
• Supervise and document delivery of care by the Home Health Aide.

Full-Time

Schedule: Friday through Tuesday 8am to 5pm

What You’ll Do:

• Obtain data and assess necessary information from the patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re‑assess the patient's needs and update the plan of care
• Perform therapeutic, preventative, and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, and reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications:

• Graduate from an accredited registered nurse program
• Licensed as a registered nurse in the state of practice
• Maintains a current CPR certification
• 1 year Home Health experience required
• Valid driver’s license and proof of insurance is required
