Ambercare Homecare in Silver City, NM is seeking a Registered Nurse to join our growing team on a PRN/part-time basis. The RN will perform initial patient assessments, develop and implement care plans, and coordinate care with the patient, family, and clinical team.

Requirements include RN license in New Mexico and CPR certification, with a preference for 1+ year home health or clinical nursing experience. Valid driver’s license and reliable transportation are needed for patient visits.

Qualifications

• Graduate from an accredited registered nursing program and licensed in the state.
• At least one year clinical nursing experience (ICU/rehab/medical-surgical preferred).
• Home health experience preferred.
• Must have CPR certification.

Responsabilités

• Completes an initial assessment of patient and family to determine home care needs and performs a full physical assessment and medical history.
• Observe and report signs or symptoms indicating changes in condition and document observations in the clinical record and notify supervisor.
• Regularly re-evaluates patient nursing needs and revises plan of care as patient status changes.
• Initiates and revises the plan of care based on patient needs and nursing diagnosis.
• Develops care plans with goals and includes patient and family in planning.
• Initiates preventive and rehabilitative nursing procedures and administers medications and treatments as prescribed by the provider.
• Provides direct patient care as defined by the State Nurse Practice Act.
• Counsels patient and family to meet nursing and related needs and provides health care instruction as appropriate.
• Identifies discharge planning needs and implements prior to discharge.
• Acts as Case Manager for assigned caseload as directed by Clinical Supervisor.
• Prepares clinical notes and updates the primary provider at least every 60 days.
• Communicates with all relevant providers about patient needs and changes in condition; obtains orders as required.

What You’ll Do:

• Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
• Observe and report any signs or symptoms indicative of changes in condition of the patient and family situation. Document such reports and observations in the clinical record and notify supervisor.
• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the provider.
• Provides direct patient care as defined in the State Nurse Practice Act.
• Counsels the patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
• Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
• Prepares clinical notes and updates the primary provider when necessary and at least every 60 days.
• Communicates with all relevant providers regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives orders as required.
• Communicates with community health related persons to coordinate the care plan.
• Performs other duties as assigned.

Qualifications:

• Graduate from an accredited registered nursing program and licensed in the state
• At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
• Home health experience preferred
• Possess and maintain valid CPR certification.
• Must have reliable transportation, current driver's license and appropriate automobile insurance
