Ambercare Homecare in Albuquerque, NM is seeking an Admissions Registered Nurse for a full-time role. Schedule: Friday through Tuesday, 8am–5pm. Must have Home Health experience and knowledge of OASIS.

Qualifications

• Graduate from an accredited registered nurse program.

• Valid driver’s license and proof of insurance is required.

Responsibilities

• Obtain data and assess patient needs across medical, psychological, social, and spiritual factors and initiate interventions with the interdisciplinary team.
• Coordinate with attending physician, interdisciplinary team, and other agencies to ensure optimal care.
• Provide regular visits to re-assess patient needs and update the plan of care.
• Perform nursing procedures as directed by the physician.
• Provide support to family during clinical pronouncement of death when needed.
• Maintain accurate patient records, charts, and care plans timely.
• Supervise and document Home Health Aide delivery of care per guidelines.

What You’ll Do

• Obtain data and assess necessary information from the patient physical, psychological, social, and spiritual factors that may impact the patient and family\'s needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient\'s needs and update the plan of care
• Perform therapeutic, preventative, and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, and reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
