Job summary

Ambercare Homecare in Las Cruces, NM is hiring a Registered Nurse for a full-time role with a Monday–Friday, 8am–5pm schedule. The RN will provide scheduled home visits, assess patient needs, develop and update the plan of care, and coordinate with physicians and the interdisciplinary team to ensure high-quality patient outcomes.

The position requires graduation from an accredited nursing program, NM licensure, current CPR, and reliable transportation; home health experience is preferred.

Qualifications

• RN with licensure in NM and graduation from an accredited nursing program.
• Experience in home health or medical-surgical nursing preferred.
• Current CPR certification and reliable transportation required.

Responsibilities

• Provide visits as assigned by Clinical Services Manager, including assessment and planning of care.
• Assess data from patient and develop care plans in coordination with interdisciplinary team.
• Communicate with attending physician and other agencies to coordinate optimal care.
• Provide visits to assess patient needs and update the plan of care.
• Perform nursing procedures, including therapeutic, preventative, and rehabilitative tasks as directed.
• Provide clinical pronouncement of death and family support as applicable.
• Maintain accurate patient records and timely care plans.

RN What You’ll Do:

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Registered Nurse
  • Graduate from an accredited registered nursing program and licensed in the state
  • At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
  • Home health experience preferred
  • Possess and maintain valid CPR certification.
  • Must have reliable transportation, current driver's license and appropriate automobile insurance
