Job summary

Moments Hospice is seeking an Admissions RN to join a dedicated field team. This full-time on-the-road role supports patients and families, with a fleet car benefit including gas and insurance. The schedule is Monday–Friday 12:00 pm–8:00 pm, offering autonomy and a focus on compassionate end-of-life care.

You will evaluate patients for hospice suitability, coordinate admissions, and maintain clinical competence while delivering high-quality end-of-life care.

Qualifications

• RN license valid and current in the state
• Driver's license, auto insurance, and vehicle
• 1–2 years of nursing experience preferred
• Utmost compassion and integrity
• Licensed as RN in the state where duties are performed
• Experience in hospital, physician practice, LTC or ALF
• Strong presentation skills
• Previous sales or service experience preferred

Responsibilities

• Participation in information gathering, analysis, education and form completion aspects of the admissions process
• Evaluate patients for hospice appropriateness, e.g. medical records review as requested
• Obtain agreement for admission of referred patients and families who meet admission criteria
• Analyze data collected to facilitate/obtain admission orders
• Build effective referral source relationships to support Moments Hospice
• Maintain clinical and professional competence
• Take initiative in developing effective relationships internally with admissions and patient care resources
• Educate and address questions from patients or families concerning hospice and palliative care

Responsibilities

• Participation in the information gathering, analysis, education and form completion aspects of the admissions process
• Evaluate patients for hospice appropriateness, e.g. medical records review as requested
• Obtain agreement for admission of referred patients and families who meet admission criteria
• Analyze data collected to facilitate/obtain admission orders
• Build effective referral source relationships to support the high quality and responsiveness expectations associated with Moments Hospice
• Maintain clinical and professional competence
• Take the initiative in developing effective relationships internally with the appropriate admissions and patient care resource persons
• Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has concerning hospice and palliative care

Requirements

• Valid Registered Nurse (RN) license
• Driver's license, insurance and vehicle
• 1-2 years of nursing experience (preferred)
• Utmost compassion and integrity
• Currently licensed as a R.N. in the state where duties are performed
• Previous hospital, physician practice, LTC or ALF experience
• Strong Presentation Skills
• Previous sales or service experience preferred
