Qualifications

• Experience in nursing home admissions or related healthcare role.
• Ability to build and maintain relationships with referral sources.
• Willingness to travel to hospitals, physician offices, and referral partners.
• Experience running financials for referrals: payer review, insurance screening, benefit verification.
• Strong multitasking to manage tours, follow-up, admissions coordination, and census-building.

Responsibilities

• Develop and maintain referral relationships with hospitals, rehab centers, physicians, case managers, discharge planners, and other partners.
• Travel regularly to referral sources to strengthen relationships and generate admissions opportunities.
• Coordinate admissions from inquiry through move-in and conduct warm, professional tours.
• Review referrals' financials including payer source and verification, and identify admission opportunities.
• Collaborate with clinical, business office, and leadership teams for smooth transitions.
• Track referrals, conversions, denials, and census-related metrics.
