Job summary

Pathways Healthcare in Lawrence, MA is seeking a dedicated registered nurse for full-time home health and hospice care. You will initiate preventive and rehabilitative nursing procedures, administer treatments, assess patients, and develop individualized care plans in collaboration with physicians and the care team.

You must hold an active nursing license for the state, have 2+ years in home health or geriatrics, maintain BLS CPR, and own a reliable vehicle.

Qualifications

• Must hold an active Nursing license in the State where the position is based.
• Preferably 2+ years of nursing experience, with a focus on home health and geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.

• Salary Range: $75,000 - $90,000
