Job summary

Pathways Healthcare is a leading home health and hospice provider delivering patient-centered care in the home. Our Medical Rehabilitation Program offers an alternative to institutional rehab, helping patients heal at home with dignity.

The role is full-time, Monday–Friday, based in Westwood, MA, and focuses on delivering comprehensive nursing care, coordinating with physicians and aides, and supporting discharge planning for optimal recovery at home.

Qualifications

• Active nursing license in the state where the position is based.
• Preferably 2+ years of nursing experience with geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform comprehensive physical assessments and gather medical histories.
• Develop and revise individualized plans of care based on patient status.
• Collaborate with physicians, nurse practitioners, and teams to create Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of care plans.

Benefits & Compensation:

• Salary Range: $72,000 - $90,000
