Holton Manor is seeking an Admissions and Care Experience Navigator (Nurse) to join our team. The role focuses on coordinating services for a smooth transition for new residents and families, managing the admissions process from intake to discharge, and coordinating tours to ensure a thorough and comfortable admission aligned with individual clinical needs.

About this position:

The Admissions and Care Experience Nurse will focus on providing new patients and families with the necessary coordination of services for an optimal and seamless transition into the facility. Manages the facility’s internal admissions process from intake to discharge and is responsible for coordination of all tours and admissions, to ensure the process is thorough and the patient has a smooth admission, with a focus on their individualized clinical needs and comfort.
