Job summary

The Clare in Chicago is seeking a Hospitality-focused Admissions Nurse to admit residents to Skilled Nursing. You will work with the admissions team to ensure equipment readiness and smooth transition through the process.

You will perform pre-admission assessments and collaborate with the Clinical Consultant to align with LCS standards while ensuring regulatory compliance. The role requires an Illinois RN license and 3–5 years in admissions or related fields.

Qualifications

• Active RN license in Illinois
• 3–5 years in admissions, intake, or case management in senior living or healthcare
• Experience evaluating clinical information for placement decisions
• Familiarity with PointClickCare is a plus
• Proficiency with MS Office Suite

Responsibilities

• Clinically admit new residents to Skilled Nursing
• Ensure necessary equipment is in place to meet residents’ needs
• Support Admissions & Outreach Manager with managing all aspects of the admissions process
• Complete onsite or virtual pre-admission assessments at hospitals or care settings
• Collaborate with the LCS Clinical Consultant to ensure consistency in standards
• Ensure compliance with federal, state, and local admission regulations

Shift - 11:00am to 7:00pm, Tuesday thru Saturday

The Clare is recruiting for a hospitality focused Admissions Nurse to join our team! This position is responsible for clinically admitting residents to Skilled Nursing.

Here are a few of the daily responsibilities of an Admissions Nurse:

• Clinically admit new residents to Skilled Nursing.
• Ensure that the necessary equipment is in place to meet the residents’ needs.
• Supports the Admissions & Outreach Manager with managing all aspects of the admissions process, from initial inquiry through move-in.
• Complete onsite or virtual pre-admission assessments at hospitals and other care settings as directed.
• Collaborate with the LCS Clinical Consultant to ensure consistency in LCS standards.
• Ensure compliance with federal, state, and local regulations governing admission practices.

Here are a few of the qualifications we need you to have:

• Registered nursing license in the State of Illinois
• Minimum of 3–5 years of experience in admissions, intake, or case management in a senior living, long-term care, or healthcare setting.
• Experience in evaluating clinical information and making placement decisions.
• Familiarity with PointClickCare a plus.
• Proficiency with Microsoft Office Suite products
