Trilogy Health Services, LLC in Monticello, IN is hiring an Admissions/Medical Records Nurse (LPN/RN) to oversee admissions intake and medical records within the Nursing Department. This role emphasizes accurate initial assessments and collaborative care planning with nursing leadership.

Responsibilities include reviewing Braden Scale results, assessing new admissions, completing MDS Section GG, ensuring documentation in MatrixCare, and providing patient care with on-call rotation.

Qualificações

• Must hold and maintain a current, valid state LPN or RN license.
• Current, valid CPR certification required.

Responsabilidades

• Review Braden Scale results and implement preventative measures for high-risk residents.
• Review high-risk residents at admission; ensure care plans and CNA sheets are updated.
• Complete nursing admission assessment and baseline care planning for new admissions.
• Complete MDS Section GG functional assessment within the required timeframe.
• Provide written and oral reports to the Assistant and Director of Nursing as needed.
• Verify census events in the Electronic Health Record.
• Complete resident documentation in MatrixCare for accuracy.
• Provide patient care as needed and participate in on-call rotation.
