Trilogy Health Services is seeking an Admissions/Medical Records Nurse (LPN/RN) in Indianapolis, IN. The role oversees admissions intake and medical records within the Nursing Department, collaborating with nursing leadership to assess new residents and baseline care plans.

The position includes completing nursing admissions, MDS Section GG assessments, and maintaining accurate documentation in MatrixCare. On-call rotation and ongoing patient care are part of the responsibilities.

Qualifications

• Must have and maintain a current, valid state LPN or RN license.
• Current, valid CPR certification required.

Responsabilités

• Review Braden Scale results and implement preventative measures for high-risk residents.
• Review high-risk residents on admission and ensure orders for interventions are in place and care plans updated.
• Complete nursing admission assessment and baseline care planning for new admissions.
• Complete MDS Section GG Functional assessment within required timeframe.
• Provide reports/recommendations to Assistant and Director of Nursing as needed.
• Verify census events in Electronic Health Record.
• Complete resident documentation in MatrixCare for completeness and accuracy.
• Provide patient care as needed and participate in campus on-call rotation.
