Qualifications

Responsibilities

• Coordinate all resident admissions from hospitals, clinics, and referrals.
• Review referral information to determine admission eligibility.
• Communicate with hospitals, physicians, residents and families about placement options.
• Verify insurance coverage and payer information (Medicare/Medicaid/private-pay).
• Complete admission paperwork and ensure regulatory compliance.
• Conduct facility tours for prospects and families.
• Maintain accurate admission and referral records.
• Collaborate with nursing, therapy, social services, and business office for smooth transitions.
• Support census development and outreach activities.
• Ensure HIPAA compliance.
