Job summary

Fairacres Manor is seeking an organized Admissions Coordinator for its Skilled Nursing and Long-Term Care team. This non-clinical role manages the admissions process from referral to resident admission, ensuring a seamless experience for residents, families, hospitals, and referral partners.

The ideal candidate will have strong communication skills, knowledge of post-acute care admissions, and the ability to build relationships while supporting census growth and exceptional customer service.

Qualifications

• High school diploma or equivalent required; associate or bachelor's degree preferred.
• Minimum one year in admissions or healthcare customer service preferred.
• Experience in Skilled Nursing, Long-Term Care, rehabilitation, or healthcare admissions strongly preferred.
• Knowledge of Medicare, Medicaid, Managed Care, and insurance verification processes preferred.
• Strong interpersonal, organizational, and problem-solving skills.
• Excellent verbal and written communication abilities; proficient with Microsoft Office and EMR systems.
• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Serve as the primary contact for prospective residents, families, hospitals, and referral sources.
• Manage referrals and admission inquiries via phone, email, fax, and electronic referral systems.
• Coordinate the admission process from referral through admission.
• Verify insurance coverage and payer sources (Medicare, Medicaid, Managed Care, and private pay).
• Schedule and conduct facility tours for prospective residents and families.
• Communicate facility services, amenities, and admission requirements.
• Collaborate with facility leadership to facilitate admission decisions and ensure appropriate placement.
• Prepare and maintain admission records, referral tracking logs, and census reports.
• Develop and maintain positive relationships with hospitals, case managers, discharge planners, physicians, and community partners.
• Follow up with new residents and families to ensure a positive transition and customer satisfaction.
• Assist with census development initiatives, community outreach, and marketing activities.
• Ensure compliance with all facility policies and applicable federal, state, and payer requirements.
