Responsibilities

• Coordinate intake and admission process for new residents, ensuring a seamless transition.
• Conduct pre-admission evaluations and assessments to identify resident needs and develop care plans.
• Collaborate with healthcare professionals and family members to understand healthcare needs.
• Develop and maintain relationships with community partners and family members to ensure smooth referrals.
• Maintain accurate records and report data on admission and retention rates.
• Participate in Quality Improvement Initiatives to ensure resident satisfaction and accreditation.
