Job summary

Complete Care at Willow Creek is seeking an Admissions Director to guide patients and families through the transition into our renovated skilled nursing and rehabilitation center. You will build strong relationships with hospitals, referral partners, and the community to support census growth, while ensuring accurate admissions and documentation.

Qualifications

• High school diploma or equivalent is required.
• Experience in skilled nursing admissions preferred.
• Positive, energetic and customer-focused attitude.
• Strong communication and relationship-building skills.
• Ability to work independently and with a team.
• Basic computer proficiency.

Responsibilities

• Oversee all inquiries, admissions, and discharges while supporting census reporting.
• Coordinate the entire admissions process, ensuring documentation is completed accurately and on time.
• Communicate with nursing, the business office, and external referral sources to coordinate admissions and bed availability.
• Build and maintain relationships with hospitals, case managers, and referral partners to support census growth.
• Evaluate referrals and assist with admission decisions.
• Maintain accurate census, referral, admission, and discharge data.
• Monitor admissions activity and referral trends; provide reports as requested.
• Deliver exceptional customer service to residents, families, and referral sources throughout the admissions process.
