Job summary

Legacy Nursing is seeking an Admissions Coordinator in Administration to ensure a smooth transition for incoming residents and their families, acting as a liaison between referring personnel and the facility.

The role involves coordinating admissions via Teams, working with nursing for clinical approval, gathering financial information, and assisting with Medicaid applications while ensuring compliance with CMS and facility policies.

Qualifications

  • College degree preferred but not required.
  • Experience in Long-Term care setting preferred but not required.
  • Admissions or Marketing Experience preferred as well as working knowledge of medical and billing terminology.

Responsibilities

  • Coordinate smooth admissions and/or readmissions into the facility via Teams.
  • Work with nursing and clinical teams to approve or deny admissions and gather necessary information.
  • Communicate with residents and/or their representatives to obtain financial information and documentation.
  • Verify insurance benefits, Medicaid eligibility, and related documentation.
  • Assist families/residents with Medicaid applications and related paperwork.
  • Inform IDT team of anticipated admissions and assign rooms.

SUMMARY

The purpose of the Admissions Coordinator position is to provide a smooth transitional period for incoming residents and their family with liability and financial well-being of the facility in mind.The Admissions Coordinator will also be a liaison between referring personnel and the facility.

DUTIES AND RESPONSIBILITIES

The following represents the essential functions of the Admissions Coordinator's role. This list is intended to be a general description of duties and responsibilities; however, it is subject to change according to the needs of the facility determined by the Administrator and/or the Corporate Marketing Team.

  • Communicate with marketing team, referral sources, residents, residents' representatives, and the facility to coordinate smooth admission and/ or re-admission into our facility via Teams.
  • Work with nursing team for clinical approval or valid reason for denial. Request further information upon the request of the clinical team. Ex: wound care notes, therapy notes, respiratory notes, behavior details, etc.
  • Effectively communicate with resident's representatives to obtain necessary financial information and documentation.
  • Verify all insurance benefits, VA contract, SNF days available, Medicaid eligibility, and that the 142 has been issued within the last 30 days.
  • Communicate with families and/or residents on financial items and/or documentation needed to support the Medicaid application.
  • Assist families and/or residents in completing the Medicaid application.
  • Obtain prior authorizations and assist with recertifications as needed.
  • In the instance of private pay being the primary source, inform the family of cost and payment expectations.
  • Ensure clinical and financial approval of referrals before accepting.
  • Complete in its entirety admissions paperwork with the resident or their representative(s).
  • Document any issues with obtaining financial or clinical information as well as completion of the admissions packet. Keep facility team informed.
  • Inform the Interdisciplinary Team (IDT Team) of anticipated admission and select appropriate room assignment. Post room number in Teams.
  • Check resident's room for readiness prior to arrival: cleanliness, odors, bed made, working call light, water pitcher and overbed table present, all light bulbs working, soap and paper products stocked, walls, floors, and ceilings in good repair, etc.
  • Provide a welcome sign/ symbol (ex: balloon) so that our residents feel at home and staff can easily identify the new resident's location.
  • Provide a welcome basket and/or booklet to new residents upon admission.
  • Remind department managers to go introduce themselves and explain their role to new residents in a timely manner.
  • Check on new admission at 24-hour mark and frequently thereafter (until resident becomes acquainted with new environment).
  • Maintain open communication lines with residents and their representatives after completion of the admissions process. Help refer them to the appropriate staff member for issues or concerns. Also, a friendly call to give an update on their loved one is impactful.
  • Submit insurance applications within 10 business days of admission and submit documentation for extension of services as needed.
  • Issue ABN/ NOMNC at conclusion of services as required by CMS.
  • Upload all appropriate documents into PointClickCare and establish resident file folders.
  • Assist Business Office Managers as needed.
  • The Admissions Coordinator participates Daily QA (morning) Meetings, Monthly/ Quarterly QA, Resident Council Meetings if requested, Care Plan Meetings upon request, in-service trainings, continuing education programs, and all other meetings called by Legacy or licensing entities.
  • Member of the Interdisciplinary Team (IDT Team).
  • Participate in community/ marketing events as requested.
  • These duties are not all-inclusive but provide minimum performance expectations which will be reviewed in the evaluation process. Supervisor may edit, alter, add to, or exclude duties and responsibilities based on the needs of the facility.

  • Freedom from illegal use of drugs.
  • Freedom from use and effects of drugs and alcohol in the workplace.
  • Anyone found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment in the position.
Education and/or Experience
  • A college degree preferred, but not required.

  • Admissions or Marketing Experience preferred as well as working knowledge of medical and billing terminology.
