Job summary

UNC Health Care in North Carolina seeks an Admission/Referral Coordinator for the Inpatient Rehabilitation Center. You will oversee referrals, coordinate pre-admission assessments with care teams, and align discharge plans with estimated insurance benefits to optimize patient outcomes.

You will collaborate with unit leadership and the medical director to ensure timely admissions and comprehensive documentation, while supporting staff education on admission criteria and regulatory changes.

Qualifications

• Bachelor’s or Master’s degree and licensure in nursing or a therapy discipline.
• Two years inpatient rehab facility/unit experience.
• BLS certification is required.

Responsibilities

• Completes the pre-admission assessment of candidates referred by case managers, therapists, physicians, or external sources.
• Determines patient goals and preliminary discharge plans with consideration of insurance benefits.
• Makes recommendations for inpatient rehabilitation or other post-acute settings and communicates them to referral sources.
• Ensures pre-admission documentation is complete and accurate.
• Facilitates communication with leadership and rehab physicians regarding admissions and barriers.
• Coordinates insurance pre-authorization and access to rehab benefits with financial counselors.
• Collects data on referrals, denials, and admissions for reporting and process improvement.
• Educates referral sources and staff on admission criteria and regulatory changes.

Summary

Responsible for overseeing the referral and admission process for the Inpatient Rehabilitation Center in conjunction with unit leadership and the Medical Director

Responsibilities

• Completes the pre‑admission assessment of candidates referred by acute care case managers, therapists, physicians, or external referral sources
• Assessments include a review of the medical record, family interviews, and consultation with unit leadership and the rehabilitation physician to determine appropriateness of admission
• Determines patient/family overall goal for rehabilitation, establishes a preliminary discharge plan and determines available insurance benefits for inpatient rehabilitation
• Makes recommendations for inpatient rehabilitation or other appropriate post‑acute care settings and communicates recommendations to referral sources in a timely manner
• Ensures appropriate pre‑admission documentation is complete
• Facilitates daily communication with unit leadership and the rehabilitation physician to discuss potential admissions to the unit and any barriers that might exist including medical, clinical, financial, or social barriers
• Ensures accurate collection of referral, denial, and admission’s data
• Seeks pre‑authorization from insurance companies and communicates availability of rehab benefits to prospective admissions
• Works with financial counselors to assist our uninsured with potential access to the rehab level of care
• Ensures accurate collection of data related to the insurance authorization process
• Works with referral sources, physicians, and the interdisciplinary team to identify opportunities to improve the efficiency/effectiveness of the admission’s process, improve overall communication, and improve patient outcomes
• Provides ongoing education to referral sources and staff related to admission criteria and regulatory changes that affect the rehab level of care
• Performs other duties as assigned

EDUCATION

• Bachelor’s or Master’s degree; licensed clinician in nursing, physical therapy, speech therapy, or occupational therapy.

EXPERIENCE

• Two years Inpatient Rehabilitation Facility/Unit experience

LICENSURE/REGISTRATION/CERTIFICATION

• BLS is required
