Job summary

UNC Health is seeking a qualified professional to oversee the referral and admission process for the Inpatient Rehabilitation Center, in collaboration with unit leadership and the Medical Director.

The role involves completing pre-admission assessments, coordinating with case managers, physicians, and insurance providers, and ensuring accurate documentation and timely communications to support patient access to rehab services.

Qualifications

• Bachelor's or Master's degree; licensed clinician in nursing, physical therapy, speech therapy, or occupational therapy.
• Two years inpatient rehabilitation facility/unit experience.
• BLS is required.

Responsibilities

• Oversee the referral and admission process for the Inpatient Rehabilitation Center in collaboration with unit leadership and the Medical Director.

• Determine admission appropriateness with medical records review, family interviews, and physician consultation.
• Coordinate discharge planning and insurance benefits for inpatient rehabilitation.
• Obtain pre-authorization from insurers and communicate rehab benefits to referrals.
• Ensure accurate data collection on referrals, denials, and admissions and support process efficiency.

Summary: Responsible for overseeing the referral and admission process for the Inpatient Rehabilitation Center in conjunction with unit leadership and the Medical Director

Responsibilities:

• Completes the pre-admission assessment of candidates referred by acute care case managers, therapists, physicians, or external referral sources
• Assessments include a review of the medical record, family interviews, and consultation with unit leadership and the rehabilitation physician to determine appropriateness of admission
• Determines patient/family overall goal for rehabilitation, establishes a preliminary discharge plan and determines available insurance benefits for inpatient rehabilitation
• Makes recommendations for inpatient rehabilitation or other appropriate post-acute care settings and communicates recommendations to referral sources in a timely manner
• Ensures appropriate pre-admission documentation is complete
• Facilitates daily communication with unit leadership and the rehabilitation physician to discuss potential admissions to the unit and any barriers that might exist including medical, clinical, financial, or social barriers
• Ensures accurate collection of referral, denial, and admission's data
• Seeks pre-authorization from insurance companies and communicates availability of rehab benefits to prospective admissions
• Works with financial counselors to assist our uninsured with potential access to the rehab level of care
• Ensures accurate collection of data related to the insurance authorization process
• Works with referral sources, physicians, and the interdisciplinary team to identify opportunities to improve the efficiency/effectiveness of the admission's process, improve overall communication, and improve patient outcomes
• Provides ongoing education to referral sources and staff related to admission criteria and regulatory changes that affect the rehab level of care
• Performs other duties as assigned

EXPERIENCE

• Two years Inpatient Rehabilitation Facility/Unit experience

LICENSURE/REGISTRATION/CERTIFICATION:

• BLS is required
