Job summary

UNC Health is seeking a manager to oversee referrals and admissions for the Inpatient Rehabilitation Center, collaborating with unit leadership and the Medical Director. The role includes coordinating pre-admission assessments, determining admission appropriateness, and planning discharge and insurance benefits.

Responsibilities span communicating with referral sources, ensuring required documentation, and improving processes with the care team to optimize patient outcomes in a 40-hour onsite

Qualifications

• Bachelor’s or Master’s degree; licensed clinician in nursing, physical therapy, speech therapy, or occupational therapy.
• Two years inpatient rehabilitation facility/unit experience.
• BLS certification required.

Responsibilities

• Oversee the referral and admission process for the Inpatient Rehabilitation Center in conjunction with unit leadership and the Medical Director.
• Complete pre-admission assessments for candidates referred by various sources.
• Determine appropriateness of admission and establish preliminary discharge plans and insurance benefits.
• Communicate recommendations to referral sources and ensure pre-admission documentation is complete.
• Facilitate daily communication with leadership and physicians about potential admissions and barriers.
• Coordinate data collection for referrals, denials, and admissions and assist with insurance authorization processes.
• Work with financial counselors to assist uninsured patients with access to rehab care.
• Identify opportunities to improve admission processes, communication, and patient outcomes, and provide staff education on criteria and regulatory changes.

Summary

Responsible for overseeing the referral and admission process for the Inpatient Rehabilitation Center in conjunction with unit leadership and the Medical Director

Responsibilities

• Completes the pre-admission assessment of candidates referred by acute care case managers, therapists, physicians, or external referral sources
• Assessments include a review of the medical record, family interviews, and consultation with unit leadership and the rehabilitation physician to determine appropriateness of admission
• Determines patient/family overall goal for rehabilitation, establishes a preliminary discharge plan and determines available insurance benefits for inpatient rehabilitation
• Makes recommendations for inpatient rehabilitation or other appropriate post-acute care settings and communicates recommendations to referral sources in a timely manner
• Ensures appropriate pre-admission documentation is complete
• Facilitates daily communication with unit leadership and the rehabilitation physician to discuss potential admissions to the unit and any barriers that might exist including medical, clinical, financial, or social barriers
• Ensures accurate collection of referral, denial, and admission’s data
• Seeks pre-authorization from insurance companies and communicates availability of rehab benefits to prospective admissions
• Works with financial counselors to assist our uninsured with potential access to the rehab level of care
• Ensures accurate collection of data related to the insurance authorization process
• Works with referral sources, physicians, and the interdisciplinary team to identify opportunities to improve the efficiency/effectiveness of the admission’s process, improve overall communication, and improve patient outcomes
• Provides ongoing education to referral sources and staff related to admission criteria and regulatory changes that affect the rehab level of care
• Performs other duties as assigned

• EDUCATION:
• Bachelor’s or Master’s degree; licensed clinician in nursing, physical therapy, speech therapy, or occupational therapy.
• EXPERIENCE
• Two years Inpatient Rehabilitation Facility/Unit experience
• LICENSURE/REGISTRATION/CERTIFICATION:
• BLS is required
