Job summary

UNC Health is seeking a referral and admission coordinator for the Inpatient Rehabilitation Center. You will oversee the referral and admission process in partnership with unit leadership and the Medical Director.

Responsibilities include completing pre-admission assessments, reviewing medical records, determining rehab goals and discharge plans, and coordinating with insurers to obtain authorization.

Qualifications

• Bachelor's or Master's degree; licensed clinician in nursing, PT, ST, or OT.
• Two years Inpatient Rehabilitation Facility/Unit experience.
• BLS certification is required.

Responsibilities

• Completes the pre-admission assessment of candidates referred by case managers, therapists, physicians, or external sources.
• Assesses medical records, family interviews, and consults with leadership to determine admission.
• Determines rehab goals, discharge plan, and available insurance benefits for inpatient rehab.
• Makes recommendations for inpatient rehab or other post-acute care and informs referral sources.
• Ensures pre-admission documentation is complete and coordinates with teams daily.
• Seeks pre-authorization and works with financial counselors to assist uninsured patients.
