Job summary

Care Network in Nebraska is seeking an Admissions Coordinator to lead the admissions process for new residents. You will coordinate admissions, communicate with patients and families, and conduct facility tours to ensure smooth intake.

The role requires self-motivation, strong communication, and prior admissions experience in a SNF or similar setting. You will generate referrals, manage reports, and maintain knowledge of payer sources in a professional team environment.

• Previous experience in admissions in a SNF or similar

Responsibilities

• Coordinates and facilitates appropriate admission to our facility.
• Maintain contact with patients/families regarding potential admissions
• Conduct interviews and tours of the facility
• Verify Insurance
• Educates family members, clients, and prospective accounts regarding services and programs offered by our facility
• Generates new referrals by establishing relationships with social workers, case managers, and other appropriate professional contacts within hospital accounts or other assigned accounts
• Assists with the development and maintenance of physician relations, as assigned
• Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner
• Maintains current knowledge and understanding of client payment sources, such as HMO, commercial insurance, Medicare, Medicaid, etc.
• Completes weekly referral log summary, monthly nursing summary, monthly referring facility summary, and other required reports
• Conducts business in a professional manner including appearance, ethical behavior, and appropriate communication skills
