Job summary

Elevate Care is seeking a detail-oriented intake/admissions coordinator to join our Chicago team. In this key role, you will manage the admissions process from start to finish, guiding prospective residents and their families with compassion and precision in a fast-paced healthcare setting.

You’ll ensure compliance, coordinate with physicians and insurance, maintain accurate records, and provide information on services and care options.

• Previous Experience in Admissions or Intake Coordination (Preferred, but not required).
• Strong Organizational & Time Management Skills.
• Excellent Communication & Customer Service Skills.

Responsibilities

• Manage Admissions Process – Guide new residents and families through the intake process from start to finish.
• Ensure Compliance – Ensure all required documentation and information are completed for admissions.
• Communicate with Families & Physicians – Coordinate with physicians, insurance companies, and families to arrange admissions and transfers.
• Handle Documentation – Maintain accurate, up-to-date patient records and admissions files.
• Provide Information & Assistance – Answer questions about services, financial options, and care plans.
• Coordinate with Other Departments – Collaborate with nursing, social services, and other departments to ensure seamless transitions.
