Job summary

Smithtown Center for Rehab is seeking an Admissions Coordinator to join our team. This role focuses on facilitating admissions, coordinating with hospital social workers, case managers, and physicians to ensure a smooth continuum of care.

The position involves weekend coverage to monitor referrals and maintain census targets, with responsibilities including reviewing documentation, assisting in marketing efforts, and reporting daily census data.

Responsibilities

• Weekend coverage required to moniker referrals remotely
• Obtains and reviews necessary medical/financial information in order to facilitate admissions process and to ensure continuum of care.
• Reviews required admission documentation with appropriate responsible party as per regulatory guidelines.
• Responsible for accuracy of assigned work. Reports problems involving assignments to Admissions Director or Administrator.
• Assists the Admissions Director in developing and managing a marketing plan to maintain census at budgeted levels.
• Demonstrates knowledge of Medicare/Medicaid guidelines and managed insurances.
• Assists in reporting the daily census.
• Maintains communication with hospital Social Workers, Case Managers, and Physicians. Communicates admissions to nursing departments.
• Marketing in the community.
