Job summary

Smithtown Center for Rehab seeks an Admissions Coordinator to help residents and families through the admissions process. You will coordinate with hospital staff and physicians to ensure accurate, timely documentation and compliant admission workflows.

This position supports census goals, participates in marketing efforts, and requires weekend coverage to monitor referrals remotely. Excellent benefits and a positive work environment accompany this role.

Responsibilities

• Weekend coverage required to monitor referrals remotely
• Obtains and reviews necessary medical/financial information to facilitate admissions and ensure continuum of care
• Reviews required admission documentation with appropriate responsible party per regulatory guidelines
• Responsible for accuracy of assigned work; reports problems to Admissions Director or Administrator
• Assists the Admissions Director in developing and managing a marketing plan to maintain census at budgeted levels
• Demonstrates knowledge of Medicare/Medicaid guidelines and managed insurances
• Assists in compiling and reporting monthly admission and discharge data
• Assists in reporting the daily census
• Maintains communication with hospital Social Workers, Case Managers, and Physicians; communicates admissions to nursing departments
• Marketing in the community

Admissions Coordinator Responsibilities:
