Job summary

Heritage Wellness & Rehabilitation seeks an energetic Admissions Coordinator to guide residents from inquiry to arrival, collaborating with hospitals, physicians, and families to ensure timely admissions and strong customer service.

The ideal candidate is organized, personable, and patient-centered, with healthcare admissions experience and a passion for helping families navigate a major transition in their care journey.

Qualifications

  • High school diploma or equivalent is required.
  • Healthcare admissions experience preferred.
  • Experience in skilled nursing, rehabilitation, or post-acute care strongly preferred.
  • Excellent communication and customer service skills.
  • Ability to multitask in a fast-paced healthcare environment.

Responsibilities

  • Coordinate the entire admissions process from referral through admission.
  • Respond promptly to inquiries from hospitals, physicians, families, and referral sources.
  • Conduct facility tours for prospective residents and families.
  • Review referrals and screen prospective residents for admission eligibility.
  • Gather and review clinical information to determine admission appropriateness.
  • Verify insurance benefits and financial eligibility prior to admission.
  • Coordinate with hospital discharge planners, physicians, case managers, and social workers.
  • Complete all admission documentation accurately and efficiently.
  • Prepare resident rooms and coordinate admission logistics with nursing staff.
  • Ensure a smooth and welcoming admission experience for residents and families.
  • Build and maintain strong relationships with hospitals and referral partners.
  • Participate in marketing activities and community outreach to support census growth.
  • Maintain accurate records within electronic admissions systems.
  • Assist with daily census management and occupancy goals.
  • Address resident and family concerns professionally and promptly.
  • Participate in morning clinical and admissions meetings.
  • Promote the facility's services and communicate the value of Heritage Wellness & Rehabilitation to referral sources and prospective residents.

Position Summary

The Admissions Coordinator is responsible for coordinating the admissions process from initial inquiry through resident arrival. This role works closely with hospitals, physicians, case managers, social workers, families, and internal clinical teams to ensure timely admissions while supporting census growth and providing outstanding customer service.

Primary Responsibilities
  • Coordinate the entire admissions process from referral through admission
  • Respond promptly to inquiries from hospitals, physicians, families, and referral sources

  • Skilled nursing, rehabilitation, long-term care, hospital, hospice, assisted living, or healthcare marketing experience strongly preferred
  • Excellent communication and customer service skills
  • Strong organizational and time management abilities
  • Experience working with insurance verification and healthcare documentation preferred
  • Comfortable building relationships with physicians, hospitals, and community partners
  • Ability to multitask in a fast-paced healthcare environment
  • Professional appearance and positive attitude
