Job summary

Eastern Iowa Rehab Hospital seeks an Admissions Coordinator to screen admissions, coordinate with departments, and manage the waitlist for residents of a LTC facility. You will assist in MDS/RAI processes, support the MDS Coordinator and Social Worker, and address barriers that affect resident placement and care planning.

You will work with Elder care teams to ensure timely, compliant admissions and to maintain high standards of patient care and documentation. Strong communication is essential.

Qualifications

  • Education: High School Diploma required; some college preferred.
  • Minimum of 2 years admitting and/or medical insurance experience.
  • Experience with patient admissions and understanding insurance benefits.
  • Proficiency with standard office software and written/spoken English.

Responsibilities

  • Interviews and assesses Elders and family, caregivers, or legal representatives.
  • Determines and arranges internal/external services for admissions.
  • Participates in case reviews and coordinates with healthcare teams.
  • Performs Medicaid screenings as indicated for NHP.
  • Maintains MDS accuracy and supports MDS Coordinator and Social Worker.
  • Review applications, manage waitlist, and assist with patient placement.

What we're looking for

The Admission Coordinator screens admissions, collaborates with other departments and facilities, manages waitlist, fields questions and completes preadmission paperwork and requirements. The Admission Coordinator assists in the coordination of all aspects of MDS and RAI processes by supporting the MDS Coordinator and Social Worker. Investigates, assesses, and initiates interventions to help Elder's cope with social, emotional, economic, and environmental challenges associated with living in a LTC facility.

How you'll contribute

An Admission Coordinator who excels in this role:

  • Interviews and assesses Elders and/or Elder's family, caregivers, and/or legal representatives.

  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.

  • Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.

  • Performs Medicaid screenings as indicated for NHP.

  • Ensures each Elder is encouraged to reach his/her maximum functional ability and helps maintain these abilities.

  • Committed to the development of the staff, Elders, and families as collaborative team members. Teamwork is a prime responsibility for each individual.

  • Participate in the admission process of prospective residents in terms of their nursing needs and appropriate placement.

  • Determine potential expense associated with a potential admission.

  • Review all applications for admission, maintain waitlist, screen referrals, act as a resource to the community.

  • Complete and assure the accuracy of the MDS for all residents.

  • Maintain current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations.

  • Maintain current working knowledge of MDS changes and most updated RAI manual.

  • Attends and participates in weekly IDT and QAPI meetings.

  • Actively involved in Family Care Conferences.

  • Works closely with Facility MDS Coordinator and Social Worker.

Qualifications and Skills:
  • Education: High School Diploma required. Some college preferred

  • Minimum of 2 years of experience in Admitting and/ or medical insurance preferred.

  • Experience in handling patient admissions preferred.

  • Extensive knowledge of insurance and their benefits and admitting forms.

  • Proficiency in recording admissions reservations.

  • Management experience preferred.

  • Basic computer knowledge including Outlook, Excel, PowerPoint, and Word.

  • Must be able to read, write, and speak English.

  • Good communication skills

  • Ability to work harmoniously with other personnel.

  • Ability to deal tactfully with patients, hospital staff and the general public.

  • Med-Surg hospitals and systems (for profit and non-profit) knowledge desired.

  • Must be able to follow written and oral instructions.

  • Must maintain a professional demeanor and appearance to reflect proper image for the hospital and the board members, medical staff, community leaders and fellow employees.

  • Must have good organizational skills and analytical ability in order to interpret information, and carry out duties independently.
