Job summary

South Shore Rehabilitation in Illinois is seeking a dedicated Admissions Coordinator to ensure referrals are processed promptly and admissions are smoothly transitioned from hospital or community settings. As part of an Extended Care affiliated facility, you’ll link departments and manage the admissions flow.

Qualifications

• Experience in healthcare, human services, marketing, census development, and the admissions process.
• Strong verbal and written communication skills.
• Team-oriented with a positive and enthusiastic approach to tasks.
• Professional grooming, manners, and conduct.
• Proficiency in Microsoft Word, Excel, and email.

Responsibilities

• Assist with client inquiries and admissions procedures
• Organize and input information into databases
• Follow the facility’s admission process
• Verify primary and secondary payer sources
• If Medicare, verify the number of days available (HIQA and prior SNF)
• If PA/Pending, notify the business office regarding the application status
• Verify payment status on all new admissions
• Enter all face sheet information into the system, referring incomplete information to social services and bookkeeping for follow-up
