Responsibilities

• Communicate admissions and transitional care needs with the appropriate stakeholders involved in care.
• Understand census, quality mix, and bed management needs of the building.
• Maintain working knowledge of Federal and State regulations, referral processes and reimbursement (Managed Care, Medicare, and Medicaid).
• Spend time in hospitals meetings with potential residents and families.
