Signature HealthCARE is seeking an Admissions Coordinator to support long-term care facilities across our network in Memphis and surrounding areas. You will communicate admissions and transitional care needs with care teams and families, ensuring a smooth intake.

The role requires at least one year in long-term care, strong MS Office skills, and the ability to travel locally. You will help manage census targets and referrals while upholding confidentiality and professional standards.

Qualifications

• Minimum of one year of experience in long-term care or related industry.
• Local travel with own vehicle is required.
• Must understand census, quality mix goals and referral expectations.
• Intermediate to advanced skills in Microsoft Office Suite.
• Must maintain confidentiality and professionalism.

Responsibilities

• Communicate admissions and transitional care needs with stakeholders involved in care.
• Understand census, quality mix, and bed management needs of the building.
• Maintain knowledge of Federal and State regulations, referral processes and reimbursement.
• Spend time in hospital meetings with potential residents and families.
