Giving Home Health Care is seeking an Admissions Patient Coordinator to manage admissions and onboarding for home health services. You will coordinate physician appointments, nursing assessments, and caregiver onboarding while ensuring timely initiation of care and a positive patient experience.

Ideal candidates have 2–5 years in healthcare coordination, strong communication, and the ability to work independently in a fast-paced environment.

Qualifications

• 2–5 years of experience in healthcare coordination, admissions, patient navigation, scheduling, case management, or related roles.
• Healthcare or home health experience preferred.
• High school diploma or equivalent required.

Responsibilities

• Serve as the primary point of contact for patients throughout admissions and onboarding.
• Manage an independent caseload through admissions, authorization, and onboarding.
• Coordinate physician appointments, nursing assessments, home visits, and caregiver onboarding.
