Job summary

Astoria Healthcare Center in Sylmar, CA is seeking an Admissions Assistant to ensure a smooth, compassionate admission experience for residents and families. You will coordinate tours, respond to inquiries, and collaborate with hospital discharge planners and referral sources.

Qualifications

• 1 year of Admissions experience in a Skilled Nursing Facility (SNF) preferred.
• High school diploma or equivalent required.

Responsibilities

• Assess inquiries when necessary and respond to referrers based on protocol, communicating admission decisions and ensuring a positive admission experience for customers and families.
• Ensure an immediate response to phone, fax, and walk-in inquiries and referrals, notifying the Administrator as needed.
• Coordinate tours and sales processes with customers, families, and referrers, either personally or through an Admissions Designee.
• Conduct follow-up on all inquiries that have not yet been converted to admissions.
• Identify, train, and schedule customer service representatives.
• Ensure proper completion, signing, and distribution of paperwork in accordance with Admission Sign-In policy and procedure.
• Meet with families and customers upon admission, ensuring appropriate preparation of rooms and introductions to nursing staff on units.
• Maintain thorough knowledge of medical, social, and financial information relating to elder care.
• Work with the Administrator and appropriate staff to ensure consistent, effective admissions, discharge, and transfer meetings.
• Review performance statistics, including customer flow, pending and lost inquiries, denials, hospital discharges, and sales activity.
• Facilitate sales opportunities for referring hospitals by identifying, initiating, nurturing, and maintaining contact with physicians, social workers, discharge planners, and other professionals.
• Stay abreast of services available within the facility to serve as a liaison with hospital discharge planners, utilization review nurses, physicians, and other professionals.
• Provide information to referral sources regarding customers' progress, adhering to HIPAA guidelines.
