Signature HealthCARE is seeking a dedicated admissions coordinator to support admissions and transitional care processes across long-term care facilities. You will collaborate with care teams, families, and external partners to align census, bed management, and reimbursement requirements, while maintaining high professional standards and confidentiality.

The role requires local travel using your own vehicle and a solid understanding of federal/state regulations governing long-term care, as well

• Minimum of 1 year experience in long-term care or related industry.

What you Need to make a Difference

• Minimum of one (1) year experience in long-term care or related industry.
• Local travel with your personal vehicle is required.
• Must understand long-term care business, including census and quality mix goals and referral expectations.
• Demonstrated intermediate to advanced skills in Microsoft Office Suite.
• Must have highest level of professionalism with the ability to maintain confidentiality.
