Job summary

Vivie in Minnesota is seeking an Admission Nurse to guide patients and their families through the admission process, conducting thorough assessments and documenting in line with Medicare regulations. You will support patient and family education, coordinate with the care team, and promote smooth transitions into care within a team-based setting. Schedule is full-time, with shift hours 8 a.m.

to 4:30 p.m. and on-site responsibilities in West St. Paul territory.

Qualifications

• RN license required in MN
• CPR certification required within six months of hire
• 1 year experience in hospice, palliative care, or end-of-life care preferred
• Valid driver's license and insured personal vehicle
• Ability to pass state mandated background check
• Ability to read, write, and speak English for staff, residents, families

Responsibilities

• Conduct Patient Assessments - Perform thorough evaluations upon admission, documenting all necessary information in compliance with Medicare regulations.
• Manage Patient & Family Relationships - Provide compassionate guidance, educate families on care options, and ensure admission requirements are met.
• Facilitate Admission Process - Complete necessary paperwork, coordinate with the interdisciplinary team, and ensure a smooth transition for patients.
• Build and Maintain Referral Partnerships - Work closely with referral sources to identify potential patients and promote Vivie’s services.
• Other Duties as Assigned - Report critical patient information to the care team to enhance quality care planning and continuity.

Schedule: Full-time | 64-80 hours/pay period | Monday to Friday | 8 a.m. to 4:30 p.m.

Pay Range: $35.34 - $46.00 / hour (non-exempt), based on experience, qualifications, and location.

Territory: Must be available to provide services in our West St. Paul territory

As an Admission Nurse, you play a vital role in guiding patients and their families through the admission process, ensuring a seamless transition into care. You’ll assess patient needs, complete thorough evaluations, and serve as a compassionate resource for those making critical healthcare decisions.

As An Admission Nurse, You Will

This Job Also Requires

• Registered Nurse (RN) license in the state of Minnesota.
• CPR certification is required within six months of hire.

• Physical capability to perform all essential job functions.
