Job summary

Valor-Hospice in Dallas, TX is seeking an experienced Admissions RN to manage admissions and provide nursing care across patient ages in their place of residence. The role requires a flexible schedule, with core hours Monday through Friday 9am–6pm and on-call duties as assigned.

You will coordinate with an Interdisciplinary Group and referring agencies to ensure comprehensive care planning and quality outcomes.

Qualifications

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State license as a Registered Nurse.
  • Two years experience as a Registered Nurse in a clinical setting; hospice experience preferred.
  • Good interpersonal skills and knowledge of regulations.

Responsibilities

  • Assess hospice patients using Nursing Assessment and LCD criteria to identify needs.
  • Develop and document the plan of care.
  • Provide nursing care with infection control measures.
  • Collaborate with Interdisciplinary Group/Team to meet patient/family needs.
  • Communicate with attending hospice physicians and other physicians.
  • Submit required paperwork timely.
  • Participate in QAPI program and hospice in-services.
  • Promote hospice philosophy and policies.
  • Meet productivity standards.
  • Perform on-call responsibilities and serve on-call as assigned.
  • Communicate effectively with patients, families, and health‑care professionals.

Job description

Primary functions are to perform admissions and administer nursing care for patients of all ages in their place ofresidence, coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency; and assume theresponsibility for coordination of care. Admission RN is a salaried position and the RN must have the ability to work a flexible schedule. Primary working hours will be Monday thru Friday 9am-6pm. Hours may vary depending on time of admission. RN will be required to hold office hours as assigned by supervisor when not assigned to admissions. RN is required to participate in on-call scheduling as assigned and other duties as assigned by supervisor.Essential Functions:Assess hospice patients utilizing the Nursing Assessment and LCD criteria to identify the physical, psycho social, andenvironmental needs.Develop and document the plan of care in correlation with AssessmentProvides nursing care utilizing infection control measures (OSHA)Work with Interdisciplinary Group/Team to ensure patient/family needs are met.Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.Timely submission of all required paperwork.Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs andmaintaining or improving competency. Participates in the Hospice’s QAPI program.Promotes the Hospice’s philosophy and administrative policies.Meets productivity standards.Performs on-call responsibilities and provides on-call services to patients and their families as assigned.Provides effective communication to patients, their family members, team members, and other health careprofessionals.Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.Licensure: Current State license as a Registered Nurse; Current driver’s license.Experience: Two (2) years experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.Skills: Nursing skills as defined as generally accepted standards of practice. Knowledge of state and federal regulations.Good interpersonal skills.Transportation: Reliable transportation and valid auto liability insurance.
