Job summary

Heart to Heart Hospice is seeking an Admissions RN to engage with patients and families, review referrals, and collaborate with the care team to assess eligibility for hospice services. You will conduct the initial patient and family meeting and obtain consents.

This full-time role emphasizes autonomy, coordination, and timely documentation, with coordination among the Patient Care Manager, physicians, and core team to ensure compassionate, compliant hospice care.

Qualifications

  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • Current State Registered Nurse License.
  • Current Driver's License, reliable transportation, and provide ongoing auto liability insurance.
  • 1–3 years RN experience in a clinical care setting, hospice or home health preferred.

Responsibilities

  • Demonstrate autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
  • Review clinical referral information and respond to requests and inquiries as appropriate.
  • Collaborate with Patient Care Manager, attending physician and hospice physician regarding eligibility and appropriateness of client for hospice services.
  • Conduct initial meeting with the patient and family over hospice service and obtain consents for care.
  • Provide the initial comprehensive assessment of immediate physical, psychological, social and spiritual needs of the hospice patient and family and initiate appropriate interventions.
  • Develop an initial care plan that establishes immediate goals and interventions, based on nursing assessment and collaborates with the attending HMD and core team.
  • Maintain regular communication with the Intake Coordinator, Patient Care Managers, RN Case Manager and core team to review patients and help with managing needs.
  • Timely submission of all documentation.
  • Performance of other duties as required.
  • Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.

What You Must Have:
