Job summary

Mount Sinai Medical Center in Miami Beach, FL seeks an Administrative Specialist in the Care Management Department to support discharge planning and post-acute care coordination.

You will handle clerical duties, communicate with providers, arrange transportation and supplies, and maintain documentation while collaborating with social workers and care managers to ensure timely patient transitions. Strong computer skills and 2–3 years of clerical experience preferred.

Qualifications

  • High School Diploma or GED preferred.
  • Extensive knowledge of computers, calculators and general office equipment.
  • 2–3 years of experience in clerical related duties.

Responsibilities

  • Initiates post-acute and community resource contacts to support discharge plans.
  • Collects and transmits patient demographic information for post-acute care arrangements.
  • Confirms arrangements with providers and documents details per policy.
  • Arranges post-acute transportation, DME and community services as directed.
  • Maintains communication with Social Worker, Case Managers and leadership.
  • Provides secretarial and clerical support including faxing, filing and typing.

Position Responsibilities:
  • Initiates telephone contacts with post-acute and community resources at the request of Care Manager/Social Worker to support patient discharge arrangements. Determines availability of services specified.
  • Collects, copies and transmits pertinent clerical and patient demographic information required to complete arrangements for post acute care and/or placement as directed.
  • Confirms post-acute arrangements with providers, communicating date/time and method of transfer. Documents communicated information according to department policy and procedures.
  • Arranges post-acute transportation, DME and community services for patient in acute care setting as directed.
  • Communicates post-acute service availability, barriers to timely discharge/transfer and issues requiring Social Work/Case manager follow-up to appropriate staff members.
  • Maintains regular communication with Social Worker, Case Managers and Case Management leadership.
  • Maintains an adequate inventory of office supplies based on usage patterns; prepares all purchase requisitions and disbursements.
  • Provides secretarial and clerical support including faxing, filling and typing.
  • Updates forms and reference files to ensure that they are current all times as requested.
  • Screens telephone calls and identifies urgency of issues initiates appropriate referrals and pages promptly.
  • Performs related clerical and record management responsibilities as required.
  • Assumes primary responsibility for the delivery and distribution of mail in the department.
  • Promotes service orientation in the performance of position duties and responsibilities and in interactions with patients, hospital staff and visitors.
Qualifications:
  • High School Diploma or GED Preferred.
  • Must have extensive knowledge of computers, calculators and general office equipment.
  • At least 2-3 years of experience in clerical related duties.
