HCA Florida Orange Park Hospital is seeking a Case Management Administrative Assistant to support discharge planning and administrative functions. The CMA will assist with post-acute placement, identify discharge barriers, and provide in-room patient support under supervision of the RN Case Manager or Social Worker.

The role requires a high school diploma (associate degree preferred), knowledge of medical terminology, and at least one year in a healthcare setting.

Job Summary And Qualifications

The Case Management Assistant (CMA) will work under the direction of the RN Case Manager or Social Worker to assist with facilitating patient discharges requiring post-acute placement or services, and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions. The CMA must be comfortable going into patient rooms.

• Healthcare professional (CNA/PCT/EMT) preferred

Looking for weekday day shift coverage - Monday, Tuesday, Thursday, Friday - 9:00AM to 3:00PM

No weekend or evening/night requirements
