Qualifications

• High School diploma or equivalent required.
• Minimum 2 years of related experience preferred.
• Must have California Class B Driver’s License and current liability insurance.
• Knowledge of office procedures and practices.
• Complete assigned tasks independently and meet deadlines.
• Interest in working with elderly and disabled populations.
• Speak clearly, use tact and discretion with patients and staff.
• Maintain confidentiality and follow dress code policy.

Responsibilities

• Assist staff with program assignments and tasks as needed.
• Coordinate with Nurse and Consultants on participant care.
• Provide personal care within the plan of care and supervise none if required.
• Assist Activity Coordinator with activity programs.
• Help prepare meals and snacks for participants.
• Clean and organize program areas and equipment.
• Record notes and complete time cards.
• Attend staff meetings and trainings.
• Coordinate transportation and maintain transport schedules.
• Document participant transport in records and ensure safe transfers.
