Job summary

Qualifications

Responsibilities

  • Assist staff with program assignments and tasks as needed.
  • Coordinate with Nurse and Consultants on participant care.
  • Provide personal care within the plan of care and supervise none if required.
  • Assist Activity Coordinator with activity programs.
  • Help prepare meals and snacks for participants.
  • Clean and organize program areas and equipment.
  • Record notes and complete time cards.
  • Attend staff meetings and trainings.
  • Coordinate transportation and maintain transport schedules.
  • Document participant transport in records and ensure safe transfers.

Job description

MISSION:

Kheir’s mission is to increase and provide access to culturally and linguistically-sensitive quality primary healthcare and human services support to the underserved and uninsured residents of Southern California.

ASSIGNMENT SUMMARY:

The Program Aide/ Driver position provides supportive services to the ADHC participants and perform administrative duties under the direction and supervision of professional staff members as assigned by the Program Director.

MINIMUM QUALIFICATIONS:
  • Minimum High School diploma or its equivalent.
  • Minimum 2 years of related experience preferred.
  • Must have current and valid California Class B Driver’s License.
  • Must have current liability insurance.
  • Knowledge of office procedures and practices.
  • Complete assigned tasks independently.
  • Make decisions within authorized boundaries.
  • Establish work priorities and meet established deadlines.
  • Must have an interest and desire to work with elderly and disabled population.
  • Speak clearly and comfortably and use tact, discretion, and sensitivity when interacting with patients; and dealing courteously and efficiently with physicians, office personnel and others.
  • Utilize discretion when handling confidential information.
  • Adhere to Kheir’s DRESS CODE policy (no open-toed shoes or sandals permitted).
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
ADHC Daily Patient Operations & Transportation:
  • Assists other staff in carrying out program assignments and assigned task as deemed necessary.
  • Works in conjunction with other staff in relation to participant care as assigned by the Nurse and Consultants.
  • Provides personal care, such a toileting, eating, transferring, dressing or grooming under the supervision of the Nurse and the Activity Coordinator.
  • Assist the Activity Coordinator in carrying out the activity program.
  • Assist other center staff in preparing meals and snack serving.
  • Cleans-up and organizes program areas, including the cleaning of equipment.
  • Write notes as indicated.
  • Makes out own time card.
  • Attends center staff meetings and in-service educational training programs.
  • Responsible for providing services and assist exercises and maintenance therapy services as trained and within his/her scope of practice as specified on the participant’s individual plan of care and documenting those services on the same day as provided.
  • Coordinate transportation services for participants and maintain a schedule of transportation of participants to and from the center.
  • Maintains records of the participant’s time spent in transit to and from the center.
  • Transport participants to and from the ADHC and assist the participants getting in and out of the transportation vehicle. Ensure that participants with cognitive and/or physical disabilities get into their homes safely.Maintain the vehicle in a safe and reliable operating condition, including arranging for regular vehicle maintenance.
  • Maintain a record tracking vehicle mileage and maintenance.
  • Closely monitor and assist with participants’ transfer and ambulation.
ADHC Team Membership:
  • Participate in bimonthly staff meetings.
  • Attend in-service trainings.
  • Report any participant-related information acquired from participants or their families to the team.
  • Other duties as assigned.
