Job summary

Sandra Eskenazi Mental Health Center (Indiana's first community mental health center) seeks a Care Coordinator to provide skills training and collaborate with the treatment team to assess clients and develop treatment plans.

You will advocate for clients, connect with community supports, and maintain timely documentation and billing, while delivering compassionate, person-centered care in a team setting.

Qualifications

• Bachelor’s degree required.
• If incumbent holds a bachelor’s of social work degree, must also possess a current, unencumbered Licensed Bachelor’s Social Worker license in the State of Indiana.
• Valid Indiana driver’s license and driving record that meets Eskenazi Health’s driving policy requirements.

Responsibilities

• Embraces, understands, and operates under the Recovery Model, including AIDET and motivational interviewing.
• Provides skills training and development to clients in group and individual settings.
• In collaboration with the treatment team, conducts client assessments and revises treatment plans.
• Provides crisis management and emergency interventions.
• Reports to treatment team in staffing and progress, barriers, and changes to care.
• Ensures timely completion of documentation and billing procedures.
